Job Title:  Investigator

Part-Time / Monday-Friday

Position Overview: Conducts investigations of alleged instances of abuse and neglect and other types of employee complaints or misconduct in a confidential manner. Role and Responsibilities: Conduct and accurately, complete a thorough investigation within Justice Center and OPWDD specified timeframe. Investigate assigned cases by various methods including, but not limited to interviews, collection of evidence, taking photographs, drawing diagrams/floor plans, etc. Analyze data and evidence to write narrative reports to effectively communicate independent conclusion of investigation to Special Review Committee. Present all investigations to the Special Review Committee. If needed, follow up on all events at the direction of the Special Review Committee to substantiate independent conclusion of investigative findings. Complete administrative requirements of the Justice Center and OPWDD investigative guidelines. Assist with audits and required follow-up. Other duties as assigned. Minimum Education and Experience Requirements (one of the following): Associate’s Degree in related field and two years’ independent investigation experience. Associate’s Degree in related field with four years’ OPWDD experience and investigation experience. A combination of education and experience that meets or exceeds the standards of the program.Qualifications and Knowledge: Familiar with OPWDD regulations and procedures. The ability to obtain and maintain an Investigator Level I Certification. Knowledge/background in investigation procedures and processes.

Job Title:  LPN   ***There is a sign-on bonus of up to $2,500 for this position***

Under the direction of a Registered Nurse (RN) assess and monitor medical concerns, provides care to ill, injured or disabled program participants.  Performs/teaches and reinforces accepted infection control and personal grooming techniques; assists in bathing, toileting, dental care, hair and nail care, personal hygiene and grooming routines as necessary.  Administers medications skillfully.  Keeps supervisor informed of individual needs and problems.  Performs other duties as assigned. Maintains confidentiality of information. Education and Experience Requirements:  Licensed Practical Nurse (LPN), valid NYS License, nursing experience with OPWDD population

Job Title: Accounts Payable/Bookkeeper

Position Overview: Perform Bookkeeping functions, supervise and assist with accounts payable. Assist Finance Director with completion of required financial reports. Assist with Finance and Human Resource related functions as directed by the Finance Director. Role and Responsibilities: Performs other duties as assigned. Maintain accurate accounts payable, enter journal entries and prepare monthly financial statements. Also assist with development and preparation of reports for internal management of agency, assist with annual audit, complete bank deposits and track purchasing to approved budget.

Education and Experience Requirements: 2 year Associates Degree in Accounting, Minimum 3-5 years’ experience in bookkeeping and accounts payable and 1-2 years supervisory experience preferred.

Job Title: Medicaid Service Coordinator

Position Overview: Provide OMRDD Medicaid Service Coordination and POCSS (Plan of Care Support Services) to children and adults with developmental disabilities to help consumers strive for the highest quality of life. Role and Responsibilities: Responsible for the development, implementation, maintenance of a consumer’s Individualized Service Plan (ISP) utilizing person centered planning philosophy through promotion of choice, individualized services and supports resulting in consumer satisfaction. Assisting the individual and/or their family in defining a person centered plan with outcomes that lead to successful inclusion into his/her community as a valued, participating member. Assuring that all referrals and associated linkages needed to achieve the valued outcomes are made. Oversee the Agency/program supports to the individual ensuring that the person’s interests, preferences, and capabilities along with inclusion into the community are maintained. Document all service coordination activities.

 Minimum Education and Experience Requirements: Associate’s degree in health or human services field and a minimum of one year working with people with developmental disabilities or a service coordinator with any population. Required Qualifications and Knowledge: Excellent verbal, written and active listening skills. Excellent organizational skills and ability to resolve crisis situations in a positive manner. Knowledge of human behavior and performance: individual differences in ability, personality, and interests. Have access to reliable transportation and possess a valid NYS Driver’s license. Must be proficient with Internet Explorer; Microsoft Excel; Microsoft Outlook; Microsoft Word; and Windows Operating System. EOE

Job Title: Administrative Assistant/Part Time

Position Overview: This position is responsible for providing professional and efficient administrative support to the department Director/Manager. Role and Responsibilities:Perform general office duties, such as answering phones, greeting visitors, ordering supplies, maintaining records management database systems, various logs, and performing basic bookkeeping work. Perform administrative tasks such as typing, filing, mailings, proofreading and data entry. Prepare memos, letters, and other documents, using word processing, spreadsheet, database, or presentation software. Open, sort, and distribute incoming correspondence, including faxes and email. Timely response to requests for information, service, and assistance. Maintains confidentiality of organization fiscal, personnel and consumer related information. Maintains a thorough working knowledge of and adheres to organization policies and procedures. Other duties as assigned. Minimum Education and Experience Requirements: High School Diploma and minimum 2 years’ experience with administrative/secretarial support. Required Qualifications and Knowledge: Ability to communicate in a concise and effective manner.  Must be confident with organizational ability and possess the capacity to multi-task and take initiative. Ability to exercise tact, discretion and confidentiality. Good verbal and written skills.

Job Title: Occupational Therapist

Assess, plan, organize, and participate in rehabilitative programs to help rehabilitate those impaired because of illness, injury or psychological or developmental problems. Essential Duties and Responsibilities:Test and evaluate patients' physical and mental abilities and analyze medical data to determine realistic rehabilitation goals for patients. Make professional recommendations on the basis of evaluation results, regarding the need for occupational therapy services. Establishes goals and objectives for each child, which are appropriate, functional, and measurable. Plans and implements appropriate treatment programs, based on the established goals and objectives, using sound treatment practices.

Education and Experience Requirements: Bachelors or Master’s Degree from an accredited college or university and must have current license to practice occupational therapy in New York State, or hold a valid temporary license pending board examination results, or pending out-of-state reciprocity completion

Job Title:  RN-Sub

Administers nursing care to ill, injured or disabled program participants.  Registered nurse (R.N.), valid New York State License.  Two to four years nursing experience.

Job Title:  Residential Counselor - Full-Time and Part-Time

Under direct supervision care for the developmentally disabled. These tasks involve encouraging, guiding and training individuals in developing daily living skills and habits, taking care of their personal needs, and maintaining the living environment. Requirements are High School Diploma and you must possess a valid driver’s license and satisfactory driving record. Must maintain all required training and certifications, including AMAP (Approved Medication Administration Personnel), CPR and First Aid certification.

Job Title:  1:1 Aide
Description: Position Overview: Under the general supervision of the Special Education Teacher, perform routine classroom assignments for the designated child. Assist with daily classroom activities and routines including facilitating and monitoring interactions of designated child. Implementation of designated child’s individual goals/objectives. and with acquisition of self-help skill and personal hygiene. Ensure a safe environment for the designated child in the classroom, playground, and on trips. Prepare for daily snack for designated child and clean up.
